About Profhilo® Structura
Profhilo® earned its place as one of the most prescribed skin boosters on the market since its 2015 launch. Structura builds on that foundation, but with a distinctly different purpose. It is the first injectable treatment with a specific clinical indication for subcutaneous fat regeneration in the face.
Developed by IBSA Derma — part of the Swiss-founded Institute Biochemique SA Group — Structura combines 45mg of high molecular weight hyaluronic acid (H-HA) with 45mg of low molecular weight HA (L-HA) using the brand’s proprietary NAHYCO thermal bonding technology. The result is a hybrid cooperative complex with a G’ prime of 95, formulated without chemical cross-linking agents.
It directly addresses two of the most common facial ageing presentations seen in clinical practice: hollowing and sagging, driven by fat compartment depletion and loss of soft tissue support.
How Profhilo® Structura Works
Structura uses the same NAHYCO technology found in the original Profhilo®. The process heats H-HA to break its existing hydrogen bonds, then cools it alongside L-HA so new bonds form between the two molecular weights. This produces a stabilized hybrid complex that can carry a high concentration of HA without compromising injectability or safety.
Once placed subcutaneously via cannula, the product spreads through the superficial fat compartments of the face. It works by restoring volume and hydration to depleted adipose tissue while simultaneously stimulating the overlying skin, delivering both structural correction and skin-quality improvement in a single treatment.
The injection protocol differs by patient presentation:
- Sinkers (Hollow, sunken appearance with lateral cheek depletion) are treated using a 22G x 50mm cannula with 1ml per side, injected into the superficial fat compartment of the preauricular area using a retrograde technique.
- Saggers (Tissue droop with loss of connective integrity) are treated using a 25G x 38mm cannula, with 0.5ml distributed across the preauricular area and zygomatic arch on each side.
Treatment requires cannula experience. Practitioners already confident with cannula techniques will find the injection protocol straightforward to integrate.

Results
The initial treatment course consists of two sessions spaced 30 days apart. Results are expected to last approximately four months, with a follow-up for potential maintenance indicated at around the five-month mark.
Clinical findings from IBSA Derma report a high patient satisfaction rate, consistent with the response observed with the original Profhilo® in the published literature.
Practitioners report that patients experiencing visible facial sinking, sagging, or volume loss — including those on GLP-1 medications — respond well to treatment. Results are comparable to certain filler outcomes, with the added advantage of ongoing tissue regeneration rather than a purely volumizing effect.
